Our office staff will verify eligibility and coverage with your insurance company for your convenience. In the event that we are unable to determine/verify benefits, coverage and eligibility, the patient will be responsible for payment of fees on the date of service. Insurance claims, benefits, and coverage information are agreed upon between patient and insurance company and are ultimately the patient’s responsibility.
